Perform utilization management clinical chart reviews. Identify and communicate opportunities to optimize policies, processes, and performance. Make recommendations for improvement to Clinical Outcomes Monitoring leadership based on current and future end-to-end processes and criteria. Accurately and thoroughly document assessment findings to accurately track, trend, and historically represent findings, gaps, deficiencies, and risks in patient care. Provide guidance and clinical expertise to clinical leaders to inform process and performance improvement. Serve as a key resource to provide explanation and information about findings, including complex/critical issues. Explain and educate on appropriate application of nationally recognized clinical guidelines. React swiftly to challenges, ambiguity and uncertainty effectively. Build and strengthen customer relationships and to find better ways to meet customer needs.
