As a Transitional Management Services Professional, you will support the people we serve in achieving their personal dreams and goals.  You will assess each person receiving services for their hopes, dreams, and desires and provide supports that facilitate achievement.  You will also help people in developing relationships in their community and with their natural supports.

To qualify, candidates must have the following: 

• A High School Diploma or equivalent is required.
• At least 2 years of experience working with adults with a mental health and/or substance use diagnosis.
