Job Description
Position:
Adolescent Female Group Home - ASSOCIATE PROFESSIONAL
Job Duties:
Assists in assessing consumer strengths and needs.
Assists in program planning
Performs quality assurance checks and documentation ensuring medical record guidelines are followed and specific program documentation is followed Supervises service provision ensuring all services and programs are delivered as specified in the plan, progress is recorded accurately and immediately, all consumer rights policies are followed, and consumer is treated with dignity and respect
Assists staff with evaluating progress
Develop task analyses as identified by the LME/County Program
Services as substitute staff as deemed necessary in emergencies
Serves as primary liaison with Case Coordinators in LME/County Programs serve. Provides quarterly written progress report to Case Coordinator on each consumer served
Maintain regular contact with consumers, staff, families, and other professionals supporting each consumer.
Provides orientation and ongoing training to staff
Job requirements:
(l) Graduate of a college or university with a master's degree in a related human service field with less than one year of full-time, post-graduate accumulated MH/DD/SAS experience with the population served. Upon hiring, an individualized supervision plan shall be developed and supervision shall be provided by a qualified profession with the professional with the population served until the individual meets one year of experience or;
- Graduate of a college or university with a baccalaureate degree in a related human service field with less than two years of full-time, post-baccalaureate accumulated MH/DD/SAS experience with the population served. Upon hiring, an individualized supervision plan shall be developed and supervision shall be provided 'by a qualified profession with the professional with the population served until the individual meets two year of experience; ;
- Graduate of a college or university with a baccalaureate degree in a not related human service field with less than four year of full -time, post-baccalaureate accumulated MH/DD/SAS experience with the population served. Upon hiring, an individualized supervision plan shall be developed and supervision shall be provided by a qualified profession with the professional with the population served until the individual meets four year of experience; and/or (4) a register nurse who is licensed to practice in the State of NC by the NC Board of Nursing and has less than four years of full-time accumulated experience in psychiatric mental health nursing.
