Position Summary

Community Free Clinic is seeking a compassionate, organized Clinical Care Coordinator (LPN, RN, or CNS) to support patient care, care coordination, chronic disease management, and clinic operations. This position plays an essential role in helping uninsured and underserved patients access high-quality healthcare while working collaboratively with providers, staff, volunteers, and community partners.

State Loan Repayment Opportunity for Registered Nurses

Community Free Clinic is an eligible site for North Carolina's nurse workforce initiatives. Qualified Registered Nurses or Clinical Nurse Specialists with educational loan debt may be eligible for up to $40,000 in state-funded student loan repayment assistance through the North Carolina Office of Rural Health. Eligibility requirements are determined by the program and applicant qualifications. The incentive is available to eligible RN candidates and is a valuable opportunity to reduce student loan debt while serving patients in an underserved community.

Essential Duties and Responsibilities

  • Coordinate patient care and follow-up services.
  • Assist providers with patient visits and clinical workflows.
  • Conduct patient intake, assessments, and documentation.
  • Support chronic disease management and patient education initiatives.
  • Coordinate referrals, specialty care appointments, diagnostic testing, and community resources.
  • Maintain accurate patient records within the electronic medical record system.
  • Assist with medication management, care plans, and patient communication.
  • Participate in quality improvement initiatives and care coordination activities.
  • Collaborate with physicians, volunteers, staff, and community partners to improve patient outcomes.
  • Support clinic programs, outreach activities, and special projects as assigned.

Minimum Qualifications Licensed Practical Nurse (LPN), Registered Nurse (RN), or Clinic Nurse Specialist (CNS)

  • Current North Carolina nursing license in good standing.
  • Experience in primary care, community health, care coordination, chronic disease management, or related healthcare settings preferred.
  • Strong organizational and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to serving uninsured, underinsured, and vulnerable populations.

Preferred Qualifications

  • Bilingual English/Spanish.
  • Experience with Athenahealth or other electronic medical record systems.
  • Experience working in nonprofit, community health, public health, or free clinic settings.
  • Knowledge of community resources and social determinants of health.
