The Housing Specialist-Community Health Worker will be responsible for oversight of all housing initiatives, pilots and research in Florida to improve linkages to affordable housing for our Medicaid members. This role will be responsible for developing and implementing the health plan’s housing strategy for the Medicaid population.  This includes developing partnerships with supportive and affordable housing providers and assisting individuals in securing accessible and affordable housing.

If you are located in Florida, you will have the flexibility to telecommute* as you take on some tough challenges.  

Primary Responsibilities:

  • Research, establish, and maintain a list of available housing opportunities in the region
  • Develop relationships with and becomes knowledgeable about local community resources including transitional housing, shelters, and permanent housing
  • Develop individualized housing plan, communicate the plan and housing opportunities to the member to ensure effective service and all placements are consistent with the members overall care plan
  • Assist members with their housing search, including applying for apartments, negotiating with potential landlords, filling out rental applications
  • Assist members in understanding tenant rights and responsibilities
  • Assist members with talking to their landlord by being a mediator if there is a disagreement or concern
  • Conduct follow-up meetings with members after housing placement to assist with issues
  • Maintain accurate documentation of service objectives, outcomes, and other member related information in the appropriate data systems
  • Participate in collaborative team meetings to provide input, support, and brainstorm solutions to common areas of concern and develop best practices
  • Assist members with in-person assessments or housing tenancy
  • Perform other duties as assigned or required

Required Qualifications:

  • High School Diploma/GED (or higher) 
  • 3+ years of experience managing needs of complex populations (eg. Medicare, Medicaid)
  • 3+ years of experience working within the community health setting in a health care role
  • 1+ years of community case management experience coordinating care for individuals with complex needs
  • 1+ year of experience with the homeless population and housing
  • Reside in the state of Florida

Preferred Qualifications:

  • Minimum 1-2 years of experience in healthcare, social work, public health, physiology, or other related field, OR bachelor’s degree in healthcare
  • Minimum 1-2 years of relevant experience in social work, case management, working with unhoused and unsheltered individuals/families 
  • Experience in a social service setting with working knowledge of case management systems and planning techniques
