Intensive In-Home (IIH) Team Lead – Charlotte, NC
Licensed Clinician (LCSW / LCMHC / LMFT)
Pathways to Life, Inc. is seeking an experienced and highly accountable IIH Team Lead to provide leadership and clinical oversight for our Intensive In-Home (IIH) program in the Charlotte area. This is a key leadership role for someone who already understands IIH, knows what makes programs succeed, and wants to lead a stable, well-supported team — not walk into a crisis.
This role exists because the program is strong enough to need leadership, not because it’s falling apart. You’ll lead a small, focused IIH team with manageable workloads, strong organizational support, and the opportunity to shape program quality, staff success, and family outcomes.
You’ll oversee a small IIH team designed to maintain reasonable caseloads and sustainable service delivery. This role is structured to ensure quality care, staff retention, and program stability, not burnout or overload.
Clinical Oversight
- Review and approve assessments, treatment plans, progress notes, and clinical documentation
- Catch documentation or compliance risks before audits do
- Monitor authorizations and continuity of care so services do not lapse
- Maintain fidelity to NC Medicaid IIH requirements, LME/MCO standards, and Pathways to Life policies
Supporting QPs
- Provide weekly individual and group supervision
- Set clear expectations around productivity, documentation, and clinical quality
- Offer early coaching and guidance when issues arise — before they become disciplinary
- Support onboarding, training, professional development, and long-term staff success
Program Stability & Quality Assurance
- Maintain IIH compliance and clinical excellence across the program
- Participate in audits and record reviews with leadership support
- Help leadership determine when and how the program can responsibly grow
- Strengthen workflows, improve outcomes, and reinforce sustainable team operations
External Communication & Collaboration
- Serve as a primary clinical contact for families, referral sources, schools, and community partners
- Coordinate care and problem-solve when escalation is needed
- Support referrals and intakes as capacity allows
Collaborate closely with Clinical Directors, HR, and Operations leadership

Required Qualifications
- Master’s degree in Human Services field (Social Work, Counseling, Psychology, or related)
- Fully Licensed or Associate-Level Licensed in North Carolina (LCSW/LCSWA, LCMHC/LCMHCA, LMFT/LMFT-A, or equivalent)
- Minimum 2 years of IIH experience (required)
- Supervisory or lead experience strongly preferred
- Working knowledge of NC Medicaid IIH service definitions and requirements
- Valid NC driver’s license and reliable transportation
- Ability to pass background check and credentialing requirement
Preferred Qualifications
- Prior experience leading or launching IIH teams
- Strong documentation and audit readiness skills
- Experience working with children, adolescents, and families
- Ability to manage performance, accountability, and deadlines in a fast-paced environment
